Franchise vs independent home care agency: the honest comparison

What the franchise fee buys

A brand clients recognize, a training system, template policies and marketing, a protected territory, and a network of owners who have made your mistakes already. Priced from the filings: Home Instead's fee is $54,000 with a total initial investment of $92,640–$350,550 (2026 FDD, Item 7); Visiting Angels charges $51,950–$89,950 by territory with a total of $125,460–$171,150. For a founder with no healthcare background, the system is genuinely the product — the question is only whether it is worth its price to you.

What it costs every year after

Royalty plus advertising fund: Visiting Angels publishes 3–3.5% + 2–2.5% of gross revenue — call it 5–6% of every dollar, forever. On an agency grossing $500,000 a year (a planning example, not a promise), that is about $27,500 a year, ~$275,000 over a decade, on top of the opening investment. An independent spends that money — or doesn't — on its own marketing and systems, and keeps the difference in slow years.

What a franchise does not do

It does not hold your license, pass your survey, employ your administrator, or open a closed door. State verdicts apply to franchisees identically — a New York need presumption or a California moratorium does not care whose logo is on the door. Due diligence order matters: state verdict first, franchise decision second.

The decision rule we would use

If the territory's referral sources already trust the brand and you have no industry experience, the franchise premium can be rational — price it as tuition plus marketing, over ten years, against the independent column. If you have healthcare experience and local referral relationships, the independent column usually wins on arithmetic. Either way, run your state through the checker and your opening through the calculator before any franchise discovery call.

Run the numbers for your state

Guides are general; verdicts are specific. The license checker applies your state's rules to your service mix, and the cost calculator prices your opening from the same dataset this guide cites.

The paperwork, in filing order

The free checkers give you the verdict and the number. The Kit gives you the working papers: your state's application packet walked through in filing order, the policies-and-procedures binder spec (what your binder must contain — never a boilerplate binder), the administrator self-audit, the insurance & bond spec sheet, the Medicaid gate map, the franchise-vs-independent 10-year worksheets, and the payroll-runway planner.
