Medicaid waiver enrollment: the second gate nobody budgets for

Two doors, two keys

Your license (or registration) lets you operate. Medicaid provider enrollment lets the state pay you. They are run by different offices, on different forms, on different timelines — and the second one starts only after the first exists. Founders who plan 'we'll take Medicaid clients from month one' usually discover the enrollment clock in month two.

What enrollment involves

A provider application through the state's Medicaid system (Georgia's GAMMIS, North Carolina's NCTracks/PCS enrollment on top of the DHSR license, Texas's provider enrollment on top of the HCSSA license), ownership disclosures, background checks, a site visit in many states, and rates set by the state — you do not negotiate them.

EVV is not optional

Electronic Visit Verification — caregivers clocking in and out electronically, by app or phone — is federally required for Medicaid personal care. Georgia requires EVV live before the first Medicaid visit. Budget the system, the training and the audit trail into the Medicaid decision, not after it.

The decision, plainly

Private pay is simpler, faster and pays your rate; Medicaid is steadier volume at the state's rate with the state's paperwork. Many agencies start private and add Medicaid once the license and cash flow are real. The verdict checker prints your state's Medicaid route inside your verdict when you pick the Medicaid lane.

Run the numbers for your state

Guides are general; verdicts are specific. The license checker applies your state's rules to your service mix, and the cost calculator prices your opening from the same dataset this guide cites.

The paperwork, in filing order

The free checkers give you the verdict and the number. The Kit gives you the working papers: your state's application packet walked through in filing order, the policies-and-procedures binder spec (what your binder must contain — never a boilerplate binder), the administrator self-audit, the insurance & bond spec sheet, the Medicaid gate map, the franchise-vs-independent 10-year worksheets, and the payroll-runway planner.
